If you take medication on a long-term basis, you can ask for a repeat prescription.
Please tick off the items needed on your repeat form and put this in the box provided in reception.
This needs to be done 2 working days before they are needed.
We are sorry but requests for repeat prescriptions cannot be taken over the telephone line. This increases congestion and increased the possibility of error.
If you have lost, or do not have a repeat form, then there are request slips available at reception. Your pharmacist may provide a repeat prescribing service for you.
Prescription requests from dentists requesting sedation
GPs should not be contacted about sedation requests before seeing a dentist. Your dentist should be prescribing for you if they think it is needed.
Questions about your prescription
If you have questions about your medicine or your prescription please contact the surgery.
Your local pharmacists can also answer any questions about your prescription. They can answer questions on medicines you can buy without a prescription.
